Why it's called Operator Error:

Every after action review eventually lands on the same finding. The gear worked. The plan worked. Somebody put the wrong thing in the wrong place at the wrong time, and nobody wants that written next to their name.

Then once in a while the wrong thing turns out to be the right thing. Light roast and dark roast in the same bag should not work as well as it does. It went in the grinder anyway, and now it has a name and a spot on the shelf.
